Having baby at the hospital my sim became pregnant after her neighbour did, my sim was immediately enormous compared to neighbour. and then went into labour before her. the neighbour had one, and popped it out in about two hours. yes, you can send sims to the hospital to give birth with just the base game. however, you'll only get to go. The only sims who are able to go to the hospital while not in the doctor sims are pregnant sims (and their significant other) when they are ready to give birth. if your sim gets sick, medicine can be ordered by phone or on the computer..
One of my favorites part of the game was watching a sim give birth. once your sim is in “labor”, they have two hours to make it to the hospital, or purchase a bassinet and give birth at home..
